
How to get your Medical Information
Follow these simple steps:
Step One: Get
copies of your medical records. In most states consumers have a right to
receive copies of their medical records. To receive copies of your medical
records you must submit a signed release of information request to your
medical provider. In some cases there may be a nominal fee for copying.
Step Two: Review the charts for discrepancies or missing
information. Each chart should state any drug allergies, previous
surgeries and any complications from surgeries, immunization records and
family history. If you determine that information is missing or incorrect,
notify your provider in writing immediately.
Step Three: Put
copies of your information into a binder. Keep this binder updated by
adding the copy of the visit charge ticket you receive after each visit.
If you belong to an HMO then you should request a findings sheet. VIA subscribers simply need to input the information into their
Personal Medical Record files or they can have their provider do so.
Step Four: If
you receive care from more than one doctor make sure each provider has the
same information on you. This can be done by printing copies of your
VIA charts forwarding the information to your provider. VIA
subscribers have access to secure email and can communicate with
confidence to their providers, pharmacies and facilities.
